Birthday Party Contract
Thank you for having your child’s birthday party with Leap of Faith Dance Company!
This contract, along with a $50 nonrefundable deposit is required to secure your birthday party booking with Leap of Faith Dance Company. The completed contract and deposit must be filled out 14 days prior to the birthday party. Reservations are not confirmed until the contract and deposit are processed by the Leap of Faith Dance Company Office Manager.
The Leap of Faith Dance Company party package includes a 2 hour party, 1 hour of dance instruction, private use of the studio, and set up/tear down time. The cost is $200 for up to 10 children. Each additional child is $10.
Every child in attendance must have an Acknowledgement of Risk, Waiver of Liability, and Media Release Form signed by a parent or guardian. Parents and children are expected to treat the studio with respect and cleanliness during their birthday party, set up, and tear down time. Failure to do so can result in an additional cleaning fee. Parents may bring decorations (with the exception of decorations that must be pinned to the wall, glitter, or confetti.) Alcoholic beverages are prohibited on studio property. Food and non-alcoholic beverages may be served during the party. However, LOFDC cannot guarantee available fridge space. Please no red drinks or red icing.
All guests are expected to behave responsibly. Children must be supervised. If necessary, the LOFDC staff member reserves the right to ask an unruly individual to leave the studio premises.
